HICAP (Health Insurance Counseling & Advocacy Program)

HICAP is a network of free community-based programs that provide help in understanding Medicare health and prescription drug benefits.

Programs for Seniors:

Assisted Living Waiver (ALW) – Available in Sacramento and Sonoma counties providing assisted living services to the elderly.

Home & Community-Based Services (HCBS) – Offers senior services in either a home or community setting for qualified individuals who would otherwise require care in a nursing facility or hospital. To apply, contact IHO (In Home Operations).  Email – IHOsaiver@dhcs.ca.gov 916-552-9105.

Program of All-Inclusive Care of the Elderly (PACE) – Provides medical and social services to enable older eligible adults to remain independent and in their homes. Available only to residents of areas with certain zip codes in Alameda. Contra Costa, San Francisco and Sacramento Counties.

Transitions For Veterans

A dedicated team of professionals, helping Veterans and Surviving Spouses determine whether they are qualified for a pension to offset the cost of Assisted Living, In-home Care, or Skilled Nursing services and assisting with the qualification process.
